Job Description

Wockhard Hospitals is seeking a Clinical Pharmacist to ensure safe and effective use of medications through clinical review, patient education, and collaboration with healthcare teams. The involves auditing prescriptions, monitoring drug therapy, and contributing to quality improvement initiatives.

Roles and Responsibilities:

  • Review and audit prescriptions toentify medication errors, dosage issues, and therapy appropriateness
  • Provide patient counseling on medications, dosage, and disease management
  • Monitor drug interactions, allergies, and adverse drug reactions (ADR)
  • Support clinicians with drug information and antibiotic stewardship
  • Participate in ward rounds and assist in medication therapy management
  • Conduct discharge audits and counsel patients on critical medications (e.g., anticoagulants)
  • Maintain and update hospital formulary and share new drug information with committees
  • Ensure proper storage and handling of high-alert, narcotic, and LASA medications
  • Conduct training sessions for nursing and pharmacy staff
  • Maintain documentation, quality indicators, and support clinical audits
