Job Description

Clinical Pharmacist - Oncology Services is responsible for managing and monitoring oncology-related medications, supporting chemotherapy protocols, ensuring safe medication practices, and collaborating with oncologists and healthcare teams to provide optimal pharmaceutical care for cancer patients. The involves reviewing treatment plans, monitoring drug interactions and side effects, counseling patients, and ensuring compliance with oncology pharmacy standards and safety guidelines.


Roles & Responsibilities:

  • Review and validate oncology prescriptions, chemotherapy orders, and treatment protocols.
  • Ensure accurate dispensing and safe handling of oncology medications and chemotherapy drugs.
  • Monitor medication dosages, drug interactions, contraindications, and treatment compatibility.
  • Coordinate with oncologists, nurses, and healthcare teams regarding patient treatment plans.
  • Provide medication counseling to cancer patients and caregivers regarding dosage, side effects, and precautions.
  • Monitor patients for adverse drug reactions and chemotherapy-related complications.
  • Support preparation and administration processes of chemotherapy and oncology medications.
  • Maintain compliance with oncology pharmacy safety standards and hazardous drug handling guidelines.
  • Ensure proper storage, labeling, and disposal of oncology medications and cytotoxic drugs.
  • Maintain accurate medication records, treatment documentation, and pharmacy reports.
  • Participate in multidisciplinary oncology discussions and patient care planning meetings.
  • Assist in developing and updating chemotherapy protocols and medication guidelines.
  • Monitor inventory levels and ensure availability of oncology medications and supportive care drugs.
  • Conduct medication reconciliation and review patient medication history for safe treatment practices.
  • Participate in oncology pharmacy audits, quality improvement initiatives, and safety programs.
  • Educate healthcare staff regarding oncology medication safety and best pharmacy practices.
  • Ensure compliance with hospital policies, NABH standards, and regulatory requirements.
  • Support pharmacovigilance activities and adverse event reporting processes.
  • Stay updated with advancements in oncology pharmacy, chemotherapy protocols, and clinical guidelines.
  • Maintain confidentiality of patient information and medical records.
  • Provide compassionate, patient-centered pharmaceutical care to oncology patients.
