Job Description
Patient Care Co-ordinator (PCCO) is responsible for guiding patients throughout their hospital journey, ensuring smooth coordination between departments, doctors, and support services. The involves assisting patients in navigating hospital processes, providing information and guidance, supporting medical documentation, and facilitating appointments, treatments, and follow-up visits.
Roles & Responsibilities:
- Accompany patients to respective counters, departments, wards, and diagnostic areas.
- Provide accurate information and guidance to patients regarding hospital services and processes.
- Respond to patient queries and assist them throughout their hospital visit.
- Review patient medical records for notes and ensure necessary information is communicated.
- Coordinate patient treatment schedules, follow-up visits, and appointments.
- Assist doctors by following instructions and supporting clinical procedures when required.
- Ensure smooth patient flow across outpatient, inpatient, and diagnostic departments.
- Help patients understand hospital processes, billing, and service procedures.
- Coordinate with nurses, doctors, and administrative staff for patient requirements.
- Maintain updated records of patient interactions and coordination activities.
- Support discharge and admission processes as per hospital guidelines.
- Assist in resolving patient concerns and escalating issues when necessary.
- Ensure high standards of patient care, comfort, and satisfaction.
- Maintain confidentiality of patient information and medical records.
- Support hospital initiatives aimed at improving patient experience and service quality.
- Perform additional patient coordination and support responsibilities assigned by management or supervisors.
